Background: Osteoprotegerin, or TNFRSF11B, is a member of the TNF receptor superfamily. This protein is a decoy receptor secreted by osteoblasts that acts as a negative regulator of bone resorption. This protein binds specifically to its ligand, the osteoprotegerin ligand, both of which are important extracellular regulators of osteoclast development. Studies of the mouse counterpart also indicate that this protein and its ligand play a role in lymph node organogenesis and vascular calcification. Alternative spliced transcripts of this gene have been reported, but their full-length nature has not been determined. Osteoprotegerin/TNFRSF11B acts as a decoy receptor for RANKL and thus opposes its function in osteoclastogenesis. This protein can inhibit osteoclast activation and promote osteoclast apoptosis in vitro. Bone homeostasis appears to depend on the local RANKL/OPG ratio. Osteoprotegerin/TNFRSF11B also plays a role in preventing arterial calcification by acting as a decoy receptor for TRAIL and protecting against apoptosis.
